To Get a Library Card:
Bring your driver's license or DMV ID and a piece of mail that you've received in the last two weeks (must show name, address and post mark) with you to the library during open hours with one of the applications below.
Each person applying for a a library card must have an application. Applications are available at the front desk of the Holbrook Public Library, or you may print one from the links below.
Persons under the age of 18 must be signed up by a parent or legal guardian, the guardian's picture ID and piece of mail will be sufficient for the child's verification.
Permission to use the internet for persons under the age of 18 is a separate form and must be requested.
If you have a library account in any of the other public libraries in Navajo County, we will use that library card for your transactions with the Holbrook Public Library.
